27th Annual RI Pharmacy Foundation Golf Outing

Registration

• 1. Golf Foursome – $680.00
  Team of four (4) registering together
  Registration as follows:
  -April ($170pp x4) = $680
  -May ($180pp x4) = $720
  -June ($190pp x4) = $760
  -July/August ($200pp x4) = $800
• 2. Single Player – $170.00
  Single player that will be placed into a foursome the day of the event. Please provide names of others who you would like to play with. Both players must request each other to be guaranteed to be placed into the same group.
  
  Single player registration as follows:
  -April = $170
  -May = $180
  -June = $190
  -July/August = $200
• 3. Event Sponsor- Diamond – $2,000.00
  Included:
  -2 Foursomes, Tee sign, golf, golf cart, breakfast and lunch.
  -Recognition on the sponsor board at the Golf Outing, upcoming Fall Seminar (prior to August 14th) and President's Dinner.
  
  -1 Complimentary Registration to the Fall Seminar OR President's Dinner. Sponsor will receive a code for registration. Please note the Seminar occurs in October.
