24th Annual RI Pharmacy Foundation Golf Outing

Registration

• 1. Golf Foursome – $600.00
  Team of four registering together
• 2. Single Player – $150.00
  Single player that will be placed into a foursome that day.
• 3. Event Sponsor- Diamond – $1,650.00
  Included: 2 Foursomes, Tee sign, golf, golf cart, breakfast and lunch. Recognition on the sponsor board at the Golf Outing, upcoming Fall Seminar (prior to August 14th) and President's Dinner.
