  1. Evaluate the impact of polypharmacy on patient outcomes and costs associated with inappropriate prescribing.

  2. Review the benefit of implementing an interdisciplinary polypharmacy service in a primary care setting.

  3. Discuss the strategies utilized within the pilot study conducted within the Integra ACO.

  4. Discuss future implementation strategies for sustained pharmacist led polypharmacy services.

  2. Interpret patient-reported home monitoring values and develop plans for modification to treatment regimens.

  3. Explain how to present remote patient monitoring programs to key stakeholders within your organization as a means to improve patient care outcomes.

REQUIREMENTS FOR REPORTING CE: Participants must login to the URI Continuing Professional Development Learning Management System at https://www.uripharmacycpd.org/ to complete evaluations and report ce participant data within 60 days following the live activity. Access codes will be given at the conclusion of the learning activity; learners must be present for the full session to receive the online evaluation access code and receive credit. Upon successful completion of the online program evaluation, cpe participant data will be automatically uploaded to the CPE Monitor and posted to the participant’s account within 72 hours.  Participants should verify their CE data was successfully transmitted to the CPE Monitor at mycpemonitor.net.
